چکیده

Varol and Ozaydin raise an important question about the effects of antihypertensive drugs and statins on arterial stiffness. In fact, antihypertensive drugs reduce large arterial stiffness, partly independently of blood pressure reduction, suggesting specific pharmacological effects on arterial remodeling. Furthermore, statins and other cholesterol-reducing agents have been shown to have beneficial effects on wave reflection and aortic stiffness reduction in several patient groups. In our study, 25 out of 39 hypertensive patients with deep intracerebral hemorrhage (ICH) and 11 out of 15 hypertensive patients with lobar ICH received antihypertensive drugs, whereas 4 patients with deep ICH and 3 with lobar ICH received statins (Table 1). Fisher test showed no significant differences between the two groups with regard to patients treated with antihypertensive drugs (P = 1, not significant) and with statins (P = 0.41, not significant), confirming our hypothesis that arterial stiffness may play a key role in the pathophysiology of deep ICH in comparison with lobar ICH, regardless of the therapy employed. Due to the small sample size of patients, the heterogeneity of the drugs administered, and the different lengths of treatments, our study design does not allow any conclusion to be drawn about the effects of these drugs on arterial stiffness. A larger number of patients are necessary to evaluate the efficacy of these promising therapeutic strategies in reducing arterial stiffness and eventually preventing deep ICH.

Aortic Stiffness in Patients with Deep and Lobar Intracerebral Hemorrhage: Role of Antihypertensive Drugs and Statins

Acampa et al. investigated arterial stiffness in patients with deep intracerebral hemorrhage (ICH) in comparison with that in patients with lobar ICH. The arterial stiffness index was significantly higher in the patients with deep ICH than in the patients with lobar ICH. This is a well-written study. However, we want to mention the associated drugs used by patients that can affect arterial stif...

Arterial Stiffness in Patients with Deep and Lobar Intracerebral Hemorrhage

BACKGROUND AND PURPOSE Intracerebral hemorrhage (ICH) accounts for approximately 10% of stroke cases. Hypertension may play a role in the pathogenesis of ICH that occurs in the basal ganglia, thalamus, pons, and cerebellum, but not in that of lobar ICH. Hypertension contributes to decreased elasticity of arteries, thereby increasing the likelihood of rupture in response to acute elevation in in...

Association between serum homocysteine and arterial stiffness: role of antihypertensive drugs

I read the article of Zhang, et al. with great interest. They investigated the association of homocysteine with arterial stiffness in Chinese community-based elderly persons. The carotid-femoral pulse wave velocity (PWV) was significantly higher in the high homocyteine group than in the normal one, however, there was no differences in carotid-radial PWV between the high homocyteine group and th...

Statin use following intracerebral hemorrhage: a decision analysis.

CONTEXT Statins are widely prescribed for primary and secondary prevention of ischemic cardiac and cerebrovascular disease. Although serious adverse effects are uncommon, results from a recent clinical trial suggested increased risk of intracerebral hemorrhage (ICH) associated with statin use. For patients with baseline elevated risk of ICH, it is not known whether this potential adverse effect...

Lobar hemorrhage in the elderly. The undiminishing importance of hypertension.

BACKGROUND AND PURPOSE We sought to determine whether hypertension is less common in primary lobar hemorrhage than intracerebral hemorrhage in other locations and whether the frequency of hypertension in lobar hemorrhage diminishes with advancing age. METHODS We identified all cases of intracerebral hemorrhage in Greater Cincinnati during 1988 by review of hospital and autopsy records as well...
